Inflammatory markers in neutropenic BSIs

The ID-IRI Study “Factors affecting inflammatory markers in neutropenic patients with bacteremia” is just published. Our study disclosed that:

CRP
Independent predictor: Hematological malignancy was significantly associated with elevated CRP (adjusted OR 4.43, 95% CI 1.23–16.43, p=0.023).
Not independently significant: Prolonged neutropenia, central line, and Gram-negative infection lost significance after adjustment, although Gram-negative infection showed a borderline association (adjusted OR 3.45, p=0.056).

PROCALCITONIN
Independent predictors: Deep neutropenia was associated with lower odds of elevated procalcitonin (adjusted OR 0.30, 95% CI 0.10–0.88, p=0.029).
Known anatomic site of infection was also associated with lower odds of elevated procalcitonin (adjusted OR 0.31, 95% CI 0.10–0.90, p=0.034).
Not independently significant: Acute lymphocytic leukemia (ALL), hematopoietic stem cell transplantation (HSCT), antibacterial prophylaxis, and Gram-negative infection were not significant after adjustment.
Multiple myeloma: Odds ratios could not be calculated because all patients had procalcitonin ≥0.5 (perfect separation).
